    School Accountability Reports
    Here you will find valuable information about individual schools across Colorado. Among many other items, you can learn about a school's academic performance, CSAP results, safety and discipline incidents, student/teacher ratio, teacher qualifications and how taxpayer dollars are spent 

    TRANSFERING STUDENTS:

    Students enrolling for the first time will be required to submit birth certificates, Social Security numbers, up-to-date immunization records and proof of residency at the time of enrollment.

    Secondary students should bring their latest report card when meeting with counselors for scheduling of classes. Elementary pupils who attended schools in other districts last year should submit their last report card from the school they last attended. The report card will be returned to the student after it has been reviewed.

    Students transferring from one middle or senior high school to another are requested to call the school they will attend and arrange to register. Failure to do so may delay enrollment.


    IMMUNIZATIONS:
    State law requires school districts to show proof that every student has had their immunization shots or is exempt from required immunization or they will not be permitted to attend. Doctor's certificate immunization history is required before enrollment of the child. If the shot record is not up to date, the student has 14 days to get the required immunizations or provide proof of complete immunization after enrollment.

    What immunizations are required?

    The following are the immunization requirements as per the Colorado Department of Public Health and Environment.

    Preschool
    DtaP 4 doses
    Polio 3
    MMR 1
    Hib 3 or 2 or 1 ^ Number of doses depends upon when doses are received
    Hepatitis B 3
    Varicella 1 - Or history of disease

    Kindergarten through Grade 4
    DtaP 5 * doses
    Polio 4 *
    MMR 2
    Hepatitis B 3
    Varicella 1 - Or history of disease

    Grades 5 and 6
    DtaP 4 + doses
    Polio 3+
    MMR 1
    Hepatitis B 3

    Grades 7 through 12
    DtaP 4 + doses
    Polio 3 + doses
    MMR 2
    Hepatitis B 3

    * If the fourth dose of DtaP and the third dose of polio were given after the fourth birthday, only four DTaP and three Polio are required.

    + Requirements for students who have been in a Colorado school prior to July 1, 2000. There may be a few exceptions with students having fewer doses. Check footnotes on the Certificate of Immunization for students new to the Colorado school system after 7/1/2000.

    Parents may sign an exemption based on religious, medical or personal reasons.

  • School Nursing and Health Programs

    Note to parents of transferring students: Colorado is a "local control" state, which means local rules and policy apply in the placement of a child moving into a Colorado district. Generally, local districts either accept the grade level of the transferring student or carry out testing of the student to place them in the appropriate grade level. Districts also have the right to apply their own cut-off dates to kindergarten age children and their own credit requirements to transferring high school students. U.S. Department of Education
